1.0 SCOPE

This method statement shall outline activity steps and sequence of work including excavation,
compaction and backfilling of soil, delivery lifting, unloading and installation of pre-cast duct banks
covered by the scope of the project.

2.0 PURPOSE

The methods and sequence of activities contained in this document shall serve as guidance and
reference to ensure proper execution of all related activities are in accordance with contract
requirement, safety & quality assurance. This method statement shall detail the work and concerns that
is to be given attention in each activity as to the following:

2.1 Before starting any activity permit to work should obtained at site.
2.2 Identification of work area, boundary and limits
2.3 Excavation of soil to required depth and dimension as shown on construction drawings
2.4 Sub-grade preparation and compaction in preparation for sub-sequent civil and/or mechanical
construction work and FDT verification.
2.5 Inspections, material sampling and testing activities required implementation of project quality
procedure to comply with Client, Owner quality requirements and ITP.
2.6 Installation of Pre-Cast Duct Banks, coating on top surface (red/green)/bituminous coating.
2.7 Fill, backfill and compaction requirements
2.8 Grading and compaction of soil required on paved areas (Asphalt Area)
2.9 Implementation of safety measures, PPE’s and compliance to HSE procedural requirements
2.10 Identification and preparation of required manpower, equipment and materials for all of the
above

6.0 DUTIES & RESPONSIBILITIES


6.1 AYTB Management
 Obtain all necessary site clearance for construction, MOCs, site takeover permits, etc.
 Ensure availability of approved and required site procedural requirements
 Ensure that project requirements and scope are fully communicated to sub-contractor
 Make available the required IFC drawings prior to start of activity and that any revisions to
approved drawings are promptly communicated to sub-contractor
 Shall oversee overall site management to ensure that responsibilities are well covered and
performed by the project organization
 Shall ensure availability of sub-contractor’s personnel, equipment and service provider’s
access to site to avoid work interruptions due to this
6.2 AYTB Project/ Construction Manager
 Shall ensure that contractual obligations are satisfied and satisfactorily performed
 Ensure that all instructions and requirements from main-contractor are within project
scope of work and agreed terms of contract
 Shall ensure timely and keen planning activities in-line with project schedule
 Ensures that key site personnel assigned on project performs their duties required
 Shall intermediate and link between departments to have a clear and continuous
communication for safety, quality and construction team
 Shall take responsibility and accountability for decisions made, interpretations of
instructions from Client and deviations from work program considering risks and
opportunities available
 Ensures availability of materials, personnel and equipments required for in all stages of the
project.
 Shall serve as only direct link to main contractor representative for all communication and
instruction to be passed to down-line of the organization.
6.3 AYTB Workgroup Supervisor
 Ensures all workgroup are having most recent revision of IFC drawings on the site
 Ensures implementation of work procedures between disciplines and workgroups
 Ensures materials, equipment and manpower required on all open & on-going work
fronts are sufficient and made available
6.4 AYTB Quality Supervisor
 Coordinates and informs workgroup supervisors of all inspection and hold-points on
open & on-going work activities
 Ensures all required inspections are promptly inform main contractor of the same
 Coordinate with third party inspection and ensure availability of technicians
6.5 AYTB Safety Supervisor
 Ensures safety procedures are in place and being strictly implemented
 Ensures all safety facilities, PPE and equipments are made available in a continuous basis
 Ensures Site safety and work permit requirements are satisfied and complied
 Oversee implementation of confined space procedures

6.6 AYTB Surveyor


 Ensures all lines and grids are true to IFC drawings
 Ensures layout are done from benchmark surveys and survey reports are maintained

6.7 AYTB Rigger


 Shall ensure safe weather condition and ample space available for crane to operate.
 Ensures stable ground for crane to operate and distance from other structures.
 Ensures crane capacity is within range for the load being lifted.
 When lifting a load above 5 Ton weight rigger 1 will be in charge in rigging activity.
6.8 Third Party Inspector and Laboratory
 Gulf Consult inspection agency is already approved for this project
 Ensures all testing, sampling and reports were accurate and performed in accordance to
Standard procedures Local or International
 Ensures that all site testing or sampling activities are logged and recorded.
 Informs schedule of laboratory tests for samples received from site
 Provides official reports for all testing performed

7.0 WORK PROCEDURE

7.1 Route Identification and Survey

1. Route of duct banks as shown on the drawings shall be marked on site.


2. Any obstruction and/or underground utility crossing identified after route verification shall be to
client attention for necessary action.

7.2 Excavation for Duct Banks

1. Conduct survey to mark locations, grids and layout of the road that to be close partially.
2. Road Crossing or asphalt paved, mechanical equipment shall be use asphalt/concrete cutter &
backhoe to remove the existing pavement. Asphalt/Concrete cutter will cut to desire depth and
remove by the backhoe and excavate up to require depth and the rest will do manually.
3. Excavate the soil manually to hardcore depth and to the required dimensions as shown on IFC
drawings.
4. Excavated material shall not be left stockpiled above the excavated pits to avoid risk of
collapsing /caving-in to the pits. All stockpiles shall be regularly moved away from the
excavation areas by at least 1.0m distance.
5. While workers are inside the pits, no vibratory activities and/or heavy equipment operations
shall be performed on adjacent areas above the pit as these will increase the risk of soil cave-ins.
6. When soft and compressible soil is encountered at footing grades, the soil shall be removed and
replaced with compacted structural fill.
7. Where excavation is to be done adjacent to existing structures, excavation shall not be deeper
than the lowest elevation of adjacent existing foundations unless underpinning requirements
have been established and performed.
8. It shall be ensured that size of excavated pit shall accommodate the required dimension of
structure as well as the working space for performing construction activities within are taken
into consideration.
9. Once required depth of excavation is attained for first half cover the trench with thick steel plate
to open for traffic. Level the soil and compact to required field density of 90% relative density if
soil is found non-cohesive or at 95% modified dry density is soil is found to be cohesive. A third-
party testing shall be required and witnessed by Client inspector for field density tests of existing
sub-grade soil.
10. Then the second half of the road shall be closed for traffic and start the cutting and excavation
same as first half is done.
11. Upon completion of excavation to the required level of second half of the road. Close the road
completely for traffic from both sides and remove steel plates already installed.
12. Compact the sub-grade using plate compactor as per line and elevation required and test using
third party witnessed by client.
13. Now the site is ready to install pre-cast duct bank in full width of the road one time.

7.3 Backfilling

1. Fill and backfill operations shall be performed after pre-cast duct bank installation work has
been completed.
2. While filling structures installed within excavated pits, the temporary shores and supports if
used shall be accordingly removed in parallel. Do not leave any unnecessary materials buried
with the pits.
3. Fill or backfill material shall not contain any deleterious materials such as waste materials, large
boulders, organic materials, construction debris, etc.
4. Filling shall be done in a proper manner so as not to damage the waterproof coating applied on
the structures or chipping and scratching the structure itself.
5. Do not compact backfilled soil directly beside the structures up to 300m distance away. These
shall be water compacted and tampered manually to avoid motion caused by force from
mechanical compactors and damage to the coating.
6. Fill and backfill activities shall be done in layers of 300mm for non-cohesive material or 200mm
layers of cohesive material.
7. Each layer of fill and backfill are to be compacted sufficiently. At every 3 lifts and at the final
layer of fill or backfill, a compaction test shall be performed with third-party inspector.
8. The final layer of fill or backfill shall follow details and elevations shown on IFC drawings making
sure subsequent construction activities are taken into consideration.
9. The top layer of fill or backfill shall follow level and grades shown on IFC drawings and any
difference shall be within allowed vertical tolerance of not more than ±15 mm.
10. All disturbed soil shall be regularly wetted with water to avoid the sand/soil particles to cause
hazards from free flying dust and fine soil particles.
11. Backfill until the existing top level of ground and compact the top layer with plate compactors.
Field density test shall be performed on the compacted top layer if the area is going to receive
further construction works (i.e. paving)
12. Reinstate the top surface of ground to its original condition is asphalt layer and all excess
earthwork or construction materials shall be taken out of the area.

7.4 Installation of Pre-Cast Duct Banks

1. Site Preparation
1.1 Prior for any lifting activities, all lifting certificates and vehicle entry permits should be
made ready on the site. All lifting equipments such as cranes, boom trucks, are all
inspected with stickers required.
1.2 Equipment operators shall have certification and ID for plant access to perform work.
1.3 Site shall be barricaded and safety signage to be posted for intended lifting activity.
1.4 The lifting radius of cranes shall be barricaded to avoid other persons entering the site
during activity execution.
1.5 The site shall be made ready to receive and to be installed. Soil sub-grade compaction
preparation shall be made available before the work.
1.6 Lifting operation shall be postponed during high winds and unfavorable weather
conditions.

2. Lifting and Unloading Operation.

2.1 Ensure lifting plan is available with the crane at site.


2.2 Crane, trailer truck position shall be ensured to be on stable ground or pavement.
2.3 Outriggers shall be provided with timber or steel pads and supported on stable soil/ground.
2.4 A certified rigger should be available on the site to work with crane operator during the
entire lifting activity. Rigger 1 will be in charge when lifting above 5 ton load weight.
2.5 No personnel’s shall be allowed to enter within the vicinity of lifting especially under the
boom area where the load is being lifted.
2.6 All lifting equipments such as hooks, shackles, slings and chains shall be checked to be in
good working condition and appropriate for the load being lifted.
2.7 Lift the duct banks into position ensuring that no plant equipments well be damage along
the route of the lift. Rigger shall be vigilant and ensure that the loads being lifted will pass
through safe path during the lifting activity.
2.8 While load is being lifted, all workers shall be at safe distance from the crane. The workers
involved with the installation activity will only approach the load during slings/shackles
installation and during removal upon unloading.
2.9 Extreme crane shall be ensured during lifting of pre-cast duct banks to avoid cracks or
damage on the structure. No impact or collision to existing structure should occur and
bringing to position should be done slowly not to drop the duct bank causing impact stress
on the concrete.
2.10 All duct banks installation shall be ensured to be in correct position and orientation in
accordance to IFC drawings. Upon setting of foundation/duct banks surveyor shall ensure
the proper position before setting in done.
2.11 Upon completion of lifting activity, all temporary barricades signage shall be taken out the
site including all debris/waste materials resulting from the work.

8.0 SAFETY PRECAUTIONS

8.1 Permit to work shall be secured prior to starting any site activity.
8.2 All workers shall comply with the requirements of the approved project safety manual, job
safety analysis and permit conditions.
8.3 All workers shall wear required PPE’s at all times while engaged on the work.
8.4 Hand tools shall be in good working conditions.
8.5 All mechanical/electrical tools used are in good conditions and inspected by SAFETY (Competent
Persons).
8.6 Cranes and boom truck third party certification shall be available with equipments.
8.7 Riggers, flagman and safety officers shall be vigilant to ensure that all lifting activity is performed
in safe manner not to damage any other facilities and plant equipments.
8.8 Crane, boom truck outrigger positions should be provided with wooden or steel pads to avoid
punching through ground, pavements or road.
8.9 Barricades shall be provided with the lifting radius and no personnel will walk or stay below
lifted load or crane boom.
8.10 All lifting accessories such as shackles, slings, chains and lifting beam when used are in good
working condition and inspected by third party.
8.11 Ensure all lifting and rigging activities postponed during presence of heavy winds and
unfavorable weather conditions.
8.12 Concrete barrier or plastic jersey filled with water to be installed along with enough signature
and blinking lights
8.13 Flagman should be always available at the site.
