AFFIDAVIT
I, MATTHEW DUGHO UMACOB, of legal age, Filipino citizen, with address
at Block 10 Lot 29 Bethlehem Street, North Olympus Subdivision, Zabarte Road,
Novaliches, Brgy Kaligayahan, Quezon City, under oath, state that:

1. Sometime on year 2014, I bought a cellphone worth Php15,000.00 (Oppo F5

brand) at SM Fairview, Quezon City;
2. I also bought a Globe SIM Card with number 0927-880-88-14;
3. Since that time, I have been using the said cellphone with the above-
mentioned mobile number;
4. With the said number, I used it to open and register an account with Gcash
and some other mobile apps such as Paymaya, Coins, Facebook, Yahoo,
Youtube, among others. I began to make transactions with Gcash and other e-
wallet apps using the said mobile number to send, receive and bank transfer
of money;
5. Sometime on October 27, 2022, the said cellphone, without my knowledge,
was stolen and snatched from my bag by a still unidentified male individual
while I was at the vicinity of my school (Far Eastern University) at Morayta,
Manila. I reported the theft and loss to the authorities at Brgy 396, Zone 41
Manila. I was really devastated with what happened. Attached is a certified
photocopy of the barangay blotter and my FEU school ID marked as Annexes
“A” and “B”, respectively, and made an integral part hereof;
6. Starting on that date, I was not able to open and access all of my mobile apps
because the One Time Pin (OTP) is being sent to the said mobile number,
much to my dismay;
7. At that time, my combined Gcash wallet balance and Piggy Bank Savings is
estimated to be Php1,300.00, more or less;
8. I want the Management of Gcash to transfer all of my wallet balance from my
registered Gcash Account with Globe SIM Card Number 0927-880-88-14 to
my newly registered Gcash Account with Smart SIM Card Number 0960-
312-0826;
9. I attest to the truthfulness and veracity of the foregoing statements.
