AFFIDAVIT FOR FORMER EMPLOYMENT

I, MARY JANE LUGOD IGUIN, of legal age, Filipino citizen,

married with postal and residence address at No. B 3 L 9 Annex A
GGH, Brgy. Malagasang 1-G, Imus City, Cavite, after having been
duly sworn to in accordance with law, hereby depose and state:

1. That, I was previously working at Aegis People Support

Center IT located at Ayala Avenue corner Senator Gil Puyat
Avenue, Makati City, as a regular employee and formerly
holding a Customer Service Representative position from
March 2014 to January 2016;

2. That, Aegis People Support Center IT was acquired by

Teleperformance Philippines in July 2014 and totally took
over in January 2016;

3. That, during my resignation sometime in January 2016, I

neither able to receive a Certificate of Employment by Aegis
People Support Center IT nor given even by the new
company which is the Teleperformance Philippines;

4. That, I am now executing this affidavit to attest to the veracity

of the foregoing facts for whatever legal intents and purposes it
may serve.
