Leyte, am unable to be present on the date scheduled for processing the documents in claiming
my wage for my 20 days of work (July 17 – August 11) under the LGU Special Program for
Employment of Students. Thereof, in my behalf, I hereby authorize my sister, Lorica C.
Superable to claim, and sign all the necessary documents relating to this matter.

Also, I am enclosing a copy of my National I.D. for your future reference.
