Proceedings of 14th National Safety Conclave-2023

Inaugural function

The 14thedition of National Safety Conclave, the annual flagship event of IQEMS,
Bhubaneswar was organized in association with Institute of Public Enterprise,
Hyderabad and Odisha State Productivity Council on 20 th-21st December at Hotel
“The New Marrion”, BBSR. The Conclave was supported by Ministry of Labour and
Employment, Govt. of India & Dept. of Labour and ESI, Govt. of Odisha. The theme
of the conclave was “Vision Zero- Mission Possible”.

Shri Vinod Kumar, IFS (Retd), Chairman, NSC-2023 gave the welcome address.

Shri Debabrata Panigrahi, CEO, IQEMS & Convenor delivered the thematic
address to the august gathering. IQEMS in collaboration with Director of Factories
& Boiler, Govt. Of Odisha is going to take up more virtual safety training
programme by using International speakers for making Vision Zero Mission
possible by which Odisha can become first state as zero fatality state. Shri
Panigrahi opined that vision Zero is commitment intended for creating &
sustaining safe & healthy work atmosphere at workplace.

The conclave was inaugurated by the Chief Guest, Sri R.S. Gopalan, IAS,
Commissioner cum Secretary, Dept. of Labour& Employee State Insurance; Guests
of Honour Shri Anupam Saha, IAS, Labour Commissioner & Director, Directorate

of Factories & Boilers, Dept. of Labour& ESI, Govt. of Odisha; Prof. Karl- Heinz
Noetel, President of Global Acting International Section of the ISSA, Germany; Shri
G. Rajesh, IFS, Director Mines, Govt. of Odisha; Distinguished Guest HE Mr. Zha
Liyou, Consul General, The Consulate General of the People’s Republic of China in
Kolkata and Guest of Honour Shri Ashok Kumar Tripathy, IAS (Retd), former
Member Board of Revenue, Govt. of Odisha. Addressing the gathering, Chief guest
Shri Gopalan said every life is precious. Someone somewhere tries to save some
time, some money resulting in accidents. But today attitude has changed due to

Hence there is need for up-skilling and skilling in industry for safety in industry.
HE Mr. Zha Liyou, Consul General, The Consulate General of the People’s Republic
of China in Kolkata said industrial safety issues are critical to life of people. We
can create platform to work together through better cooperation. Citing example
of state to provincial relationship in China, he exuded confidence that both
countries need to initiate , promote cross functional relationship. Sri Ashok
Kumar Tripathy, IAS (Retd), former Member Board of Revenue, Govt. of Odisha
underlined the need to capture near miss incidence trend & data analysis using
predictive analytic tools.

The prestigious “Kalinga Safety Excellence Award” for the assessment year 2022
was presented by Chief Guest, Sri R. S. Gopalan , IAS, Commissioner cum Secretary,
Govt. of Odisha along with Guests of Honour to the winners of 23 different mines
for excellence in the field of mines safety.

different industries for excellence in the field of Safety & Health.

Twenty eight papers were presented in eight engaging & insightful technical sessions
covering several aspects of safety by experts from different sectors including mining,
manufacturing, power, fertilizer, Oil & Gas etc.

The Key takeaways are summarized as follows:

 AI based mobile APP is being used for instant recording & sharing of
unsafe acts and observations.
 Digital QR CODE is used for reporting safety observations/near-miss
incidence instantaneously from site
 Voltage Reduction Device (VRD) need to be fitted to welding machine
during arc welding since it reduces maximum unloaded open circuit
voltage across the output terminals of welder to a safe voltage less
than 24 volts. This device protects welder from electrocution.
 Virtual Reality(VR) & Augmented Reality (AR) based training are
being extensively used which enables workers to enter a virtual
work environment that imitates the real one
 “5 S” concept i.e. Sort, set in order, shine, standardize and sustain is
being used is being used to organize work place efficiently &
effectively.
 Colour coding of tools & tackles(Rigging) is adopted at shop floor
levels to increase the confidence among workers and trace the left
out tools for inspection.
 Auto entry & exit of dispatch vehicles is possible by using RFID tag
 An electronic permit to work (ePTW) system improves safety by
capturing every aspect of the process from start to finish.
 RFID based access control helps to restrict unauthorized entry in
hazardous area as well as helps to collect information such as
employee ID, equipment no of every individual who entered into
those restricted areas.
 Suraksha Card is a safety gadget provided to personnel and using the
card, real time monitoring of personnel is done.
 surveillance by drone is used to inspect hard to reach unsafe areas &
monitor critical assets such as overhead lines , measurement of
thickness of stacks.
 Anti sleeping alarm installed in locomotive engine will alert
LOCO Pilot to avoid sleeping for smooth LOCO operation.

Sri Agam Kumar , Chief of Safety, Tata Steel Meramunduli spoke at


length about connected worker at hazardous area like blast furnace
section where there is complete mapping of person like face
identification system whereby any one getting into hazardous area
alarm is triggered. Digital technology is helping people to respond to
hazard before hazard becomes an incident. Suraksha card is provided to personnel &
using the card for real time monitoring of working personnel.

Prof. Karl-Helnz Noetel, President of global acting International


Section of the ISSA, Germany stressed on prevention focused approach
& continuous improvement practices in attaining Vision Zero goal. He
elaborated on seven golden rules as part of global initiatives of which
commitment & leadership, people involvement are important.

Sri Harshvardhan Pande , Head HSE & Sustainability, Vedanta Ltd,


Alumina Refinery, Lanjigarh emphasized on engineering control
measures like voltage reduction device( VRD) to avoid electrocution
hazard during Arc Welding.

Sri P. K. Das , GM Safety, NALCO ALUMINA Refinery , Damanjodi
shared some best practices like use of thermal infra camera for
detection of hotspots of critical equipment, Industry 4.0 based AI
system for boiler operation, online contractor safety management
and Virtual Reality(VR)& Augmented Reality (AR) based training for
employees. He explained how digital QR Code is helpful in raising safety suggestion by
any shop floor worker.

TECHNICAL SESSION II

Odisha

Dr Sc. ETH Mark Fuellemann , founder/Director Practice & experience


Ltd, Switzerland sharing his experience said majority of workmen in
steel plants work in plant operation and recent trend is outsourcing of
these jobs to contractors which is a major challenge since they have
different skills, experiences, speak different languages & have different
cultures. They have comprehensive hands on training for contractor workers ending
with test before being allowed to work onsite

Dr Surya Prakash, Sr. Professor, National Institute Of Disaster
Management , New Delhi ( Govt. of India) stressed on the services
offered by their organization in the area of training & capacity
development for various organizations . He urged organizations to
prepare disaster management plan and share with all stakeholders
which is a prerequisite.

Sri D P Bagchi, Safety Officer, Jitpur coal mines , SAIL, Dhanbad


touched upon digital initiatives like real time monitoring of mine
environment , carbon monoxide gas emissions to ensure safe
working.

TECHNICAL SESSION III

Chairman- Dr Ramesh Kumar Behera, Dy. Director, Angul Division,


Directorate of Factory & Boilers, Dept. Labour & ESI, Govt. of Odisha

Sri Devendra Kumar Swain , Chief Fire Officer, (SAG), (FPW), Govt.
of Odisha stressed on proper identification of hazard to mitigate
risk. He urged the need for possessing Fire Service Certificate for
every residential building. He advised not to close the balconies to
facilitate evacuation & rescue operation in the event of building fire incidence.

Lt. Col . B K Soni, DGM(HSE), IOCL Pipeline Division speaking on
“Behavior Based Safety” said accidents mostly caused by unsafe
behaviors. Leadership & commitment has immense potential to
modify behaviors. Leaders need to lead by example and there should
be no gap between what you say & what you do. Our belief, attitude
needs to change to modify unsafe behavior to safe behavior.

Prof. Karl-Heinz Noetel of Germany shared his thoughts on


innovative protective clothing for steel industry. Schumer Secan
fabric is used for making protective uniforms(resistant to molten
metal & heat)for use in steel & aluminium industries. Conforming to
EN standard. They last 50 or more washings at 95 degree centigrade
compared to 63 °c as per Indian Standard.

Sri A J Bhinde, DGM (F&S), Indian Oil Corporation Ltd. Paradeep
Refinery said Compliance of Intrinsically Fire Retardant (IFR)
clothing for employees is mandatory. Online monitoring
system (SAP) to report & investigate high potential Near miss incidents is in
place. Oil dispersal media is used to dilute eventuality of oil spillage from
pipeline. Drones are used for monitoring personal protective equipment (PPE).

Smt. Asha S Kartha, GM I/C (DED) gave presentation on


“Steel Manufacture Safety Risk Management”. She spoke on
ensuring barriers/ control measures for credible scenarios /
risks with provision of online detectors, neutralizing pit-etc are
maintained healthy in emergencies conditions like power
failure, gas leakage She further emphasized that
categorization of low, medium & high risks should be done properly and
consequences to environment, people & surrounding should get priority.
Ammonia unloading valves which were manual were changed to auto mode to
mitigate risks.

Sri Gagandeesp Ojha ( Head Fire, Power Safety), Vedanta


Ltd. Jharsuguda touched upon camera based surveillance
program for critical locations digitally to capture safety non-
compliance hazards are caught and reported with pictures &
video of the incident to plant head & safety team for necessary
actions. Digitization of belt conveyor guarding & interlocking system helps
when conveyor guard is opened with inbuilt hooter buzzing alarm until
conveyor guard is closed .

Mr. Grant J Wilson , HSE coordinator , Px-Projct at IOCL ,


Paradip spoke on Safe Think philosophy guiding safety
innovation. Electronic permit to work system is assessed from
dashboard. E-permit to work helps to communicate to each
area & easy to monitor jobs. They have a system of
nominating HSE champion among workforce. If someone is good in scaffolding
job, he is made scaffolding champion and rewarded appropriately. Legal
compliance is compiled every month.

Dr Sc. ETH Mark Fuellemann , founder/Director Practice &
experience Ltd, Switzerland brought new concept of migrant
workers who create diversity in skills , languages, culture and value
system and pose challenges to well being. We need to understand
their feelings & teach respect since they are discriminated at
workplace.

Sri Satyendu Jena, Manager Mining, Daitari iron ore mines shared
some of the best practices on blast induced vibration study, IOT
based emergency switches, real time monitoring of vehicles for
legal compliance & blacklisting of vehicle operator for violation of
norms, online visitor pass generation , test for intoxication and mock drill training.
Automatic entry & exit of ore dispatch vehicles is monitored by RFID tag.

Sri Tanmaya Pattnaik, Deputy Manager (Mines), JSPL, Kasia Iron Ore
Mines highlighted digital QR Code adoption for reporting of safety
observations. Specialized training are imparted on LOTO (Lock Out
Tag Out), Work at height, confined space, alcohol detection to all
workers .

Sri Aman Chopra , AGM, HPCL-Mittal Energy Ltd., Bathinda, Punjab


focused on incident& injury free(IIF) culture which is about
demonstrating daily care for one another. IIF teams focus their
efforts on the behavioral aspects of the workforce aiming to enhance
relationships and cultivate a workplace that is free from incidents and
injuries.Contract workers are considered as business partners involving them in
decision making process.

Dr A. K. Mahapatra , Director , Sum Hospital & Medical College,


Bhubaneswar dwelt on “ Patient Safety” and said Fire Safety is
major safety issue across hospitals in the country. 90% of fire
occurs due to electrical short circuit problem. This is mainly due
to overload of electric circuit due to addition of appliances. Most
of the time fire sprinkler doesn’t work which is case of
communication gap between service provider & management. Air
conditioners run 24 hours & there is no standby air conditioner, resulting in fire
incidence.

focused on ten elements of safety management culture assessment
(SMCA) in dealing with process safety management of hazardous
processes which is gap assessment to determine existing behaviors.
To promote people participation they award people with consistent
track record on safe behavior and impose punishment as well for poor safety record.

Talked about Chardham tunneling sequence in the context of
recent SILKYARA disaster in Uttarakhand . There was no real time
monitoring in place, no supervision and no escape passage for exit.

Sri Amit Kumar Parida ,Asst General Manager(Mines), NALCO


Central & North Block Mines, Damanjodi said NALCO Surakhya
mobile app enables near miss, unsafe act reporting. Training on
fire safety is imparted online. TDS for control of vehicle
movements is especially helpful during monsoon/fog.

Sri A S. Bhave, Head ( Fire & Safety),GAIL(India) Ltd. Bhubaneswar ,


spoke on offsite Mock drill is carried out in coordination with
district crisis group whose response time is 20-30 seconds only. He
further talked about critical interlock features of LNG tank
operated at 14.9 bar pressure.

Technical Session VII

Panel discussion was organized comprising of International & Indian safety experts
deliberated various emerging challenges and expressed concern over near miss
incidences increasing exponentially. To address such issues, mapping of hazardous
area is essential. Root cause analysis is a simple tool to analyse near miss & accidents.
Predictive analytic tools can predict trends/ patterns of near miss and accidents.

Prof. Karl-Heinz Noetel , President of global acting international
section of the ISSA , Germany spoke on “ The Value of Cooperation
-------- Health & Safety”. He explained through interesting video clips &
pictures how accidents can be prevented through international
cooperation. He reiterated that VISION ZERO is a movement in the entire world and it
is possible to achieve in future.

Dr T. K. Das, Tata Refractory Ltd , Krosaki, Belpahar gave some


useful tips for good health and said stress & emotion management
is most important to live healthy life. He also advised to keep five
heart care pills in pocket for heart attack prevention for people
with heart problem & chronic diabetes.

Sri Jatin David Bagh, Dy. Manager (Mining), Business Excellence,


Kodingamali Bauxite Mines, OMC Ltd. Discussed OHSE web-based
o’ SAFE portal which acts as a medium of near miss reporting ,
accident and incident investigation .Online based vehicle tracking
system is helpful in tracking speed violation by vehicle operators.

Dr S. K. Sahu, Scientist –F , Integrated Test Range , DRDO, Chandipur


highlighted safety aspects of handling explosives which are hazardous
materials. Due care has to be taken in handling (packing, stacking),
road transportation & disposal of un-detonated explosives.

Sri Ashok Kumar Sahu, Manager EHS, GMR Kamalanga


Energy Ltd shed light “on 5S technique”: sort, set in order,
shine, standardize and sustain to organize workplace for
efficiency & effectiveness. Plant is divided into several
zones and are supervised by 5 S zone leaders for effective
implementation of 5 S practices. Unnecessary items are
removed & immovable items are shifted to a “Red tag area” so that that they
are easy to remove later on. Standardizing the working zone process used to
sort, order and clean the workplace. Each department should initiate the 5s
project in their respective work area.

Sri Chandan Kumar Agarwal, AGM(Mines), Bolani ore


mines, Rourkela Steel Plant., shared best safety practices of
their unit. Height pass physical assessment facility is
available. Mobile App based incident/near miss reporting
system is in place. Alcohol breath analyzer has reduced
cases of drunken driving & consequent accidents.

Sri Sushanta Kumar Panda, ACC Limited Bargarh cement
works gave a detailed account of best safety practices in
their unit. Plant is divided into 90 zones/sub zones. Digital
QR code is activated using software and can be scanned to
observe unsafe acts and working conditions. Safety training
is imparted to workers including contractual workers in video
mode. Those who satisfactorily have completed training, stickers are pasted on
helmets for easy identification. Violation of safety rules if exceeds four times by
any one, plant entry is mandatorily stopped. High resolution CCTV camera &
drone is used for inspection of confined spaces like silo, kiln etc.

Sri Nagaraju Devarapalli, Head–EHS, Coromandel


International Limited, Vizag spoke on “Safe Start” program
aimed at creating safe behaviour mindset which aligns with
global best practices in behavioral safety. Drones are
deployed for inspection of critical assets such as overhead lines, thickness of
stack eliminating the risks associated with traditional methods.Radio
Frequency Identification (RFID) based system enables auto identification of
vehicle & turnaround time is reduced.

capacity building are the 2 corner
stone for achieving “Vision Zero”. Shri Vinod Kumar, IFS (Retd), Chairman, NSC
2023 summoned up the two day’s proceedings in full house audience and
promised to bring Latest Technology, Best Practices, Better Compliance to the
grass root. Few delegates appreciated there experience and spoke high about the
conclave. Over 200 feedback's received in written form and almost 95% gave
excellent remarks with very good suggestions. Sri Debabrata Panigrahi assured

Govt. of Odisha and promised to take this to a much bigger scale.
