SUBJECT: Investigation on/re: Re-stabbing Incident

l. AUTHORITY

ll. MATTERS TO BE INVESTIGATED / OBJECTIVES OF THE INVESTIGATION

1. To determine the killing of the victim Juan P. Dela Cruz, 30 years-old, married, of Sunrise Village,
Guiwan, Zamboanga City.
2. To identify the suspect responsible for the killing of the aforesaid victim in his possible arrest.
3. To extract witnesses and find pieces of evidence to establish probable cause and filing of
appropriate charges in court against the suspect.

lll. FACTS OF THE CASE/DETAILS OF THE INVESTIGATION

1. (D/T/P Occurrence and events); October 26, 2023 at around 7:30 in the morning along Sunrise
Village, Guiwan, Zamboanga City.

On or about 7:30 in the morning of October 26, 2023 our office received a phone call from
concerning citizen that stabbing incident occurred along at Sunrise Village particularly outside the
house of the victim Juan P. Dela Cruz and this report was officially registered under IRF FORM NO.005
Acting on the report, the investigating officer, Patrolman Prince Kenthzhar M. Omar, proceeded to the
place occurrence together with alert team on board on a police mark vehicle to verify such report.
Reaching thereat, a male person was seen laying on the ground resting right side position oozing with
blood with multiple stab wounds believe to be lifeless and identified later as Juan P. Dela Cruz
through his wife.

However, the suspect was no longer in the area believe to be hiding and identified as Chico
Omar of the same place.

Prior to the incidents, the victim and the suspect together with their two neighbors namely
Alex E. Loco and John Mark C. Christian were drinking session.

lV. INVESTIGATIVE FINDINGS

1. During the ocular inspection, recovered from the scene of the crime one kitchen knife measuring
more or less 70 inches long, one pair of slipper color green, broken empty bottles of Tanduay Rhum.
Further noted, four pieces of wooden chairs and small wood squared table, and four glasses were
scene scattered on the ground
