2.b Photocopy of Diploma;
2.c Photocopy of Transcript of Records;
2.d Photocopy of certificate of eligibility/rating/PRC license, if applicable;
2.e Certificates of relevant training, seminars and awards/recognition, if any;
2.f Performance rating in the last rating period (if applicable);
2.g Certificate of previous employment, if applicable;
2.h Certification of no pending administrative charge; and,
2.i NBI Clearance (for cases of original appointment and reemployment).
