4. Certified Copy of Technical Description

5. Certified copy of Approved Plan

6. Copy of Tax Declaration

7. Alienable and Disposable Certification from DENR

8. Lot Status from Land Registration Authority (LRA)

9. Certification from the MTC/RTC

10. Copy of Valid ID

11. Application Fee (Php. 50.00)

12. Four (4) Documentary Stamps
