This has reference to the herewith copy of the letter dated April 20, 2023 from Mr. Domingo N. Pura, Branch Manager of the National Food Authority (NFA), Albay Branch Office regarding the one-time grant of rice assistance to all government ‘employees and workers for fiscal year 2022. 2. In view of the foregoing, all Schools Division Offices (SDOs) within the province of Albay are hereby requested to prepare and directly submit the following to the Office of the National Food Authority, Albay Branch. a. Annex A - Master List of beneficiaries containing the names of the regular/contractual/casual employees and COS/JO workers based on the Agency's personnel complement/warm bodies as of November 30, 2022 (assigned in the Province of Albay only); b. Authorization letter signed by the Head of Agency for the designated focal person or representative to transact with the NFA and contact number; and c. Annex B - Acknowledgement Receipt (AR) indicating the names of qualified beneficiaries based on the submitted Master List of Beneficiaries with their affixed signatures together with the photocopy of the employee's Identification Cards (Ds). SADSAD ‘ In relation to the implementation of the One-Time Grant of Rice Assistance (OTRA) to government employees and workers pursuant to Administrative Order (AO) No.2, may we request the following based on the guidelines in the availment and issuance of rice stocks: 1. Masterlist of beneficiaries containing the names of the regular / contractual/ casual employees and COS / JO workers based on the Agency's personnel complement/ warm bodies as of November 30, 2022 (assigned in the province of Albay only) by filling-up the attached format (Annex A); 2. Authorization letter signed by the Head of Agency for the designated focal person or representative to transact with the NFA and contact number; 3. The agency-beneficiary shall prepare the Acknowledgement Receipt (AR) indicating the names of qualified beneficiaries based on their submitted Master List of Beneficiaries with their affixed signatures (Annex B). Attached photocopy of ‘employees’ Identification Cards (IDs). The AR shall also include the name and signature of the authorized representative! focal person who will withdraw the rice. The NFA Albay Branch shail inform the designated focal person on the schedule of withdrawal of stocks based on the Authority to Issue (Al) to be secured at Albay Branch Office, Pier Site, Legazpi City. The designated focal person of the agency-beneficiary shall undertake the withdrawal of rice allocation of all the qualified beneficiaries subject to the submission of the following documents: a. Special Power of Attorney (SPA) b. Acknowledgement Receipt (AR) cc. Government Issued ID of the Focal Person d. Master List of Beneficiaries ‘The NFA shall release the total rice requirements of the agency-beneficiary to the designated focal person at 50kg per sack representing rice allocation for two (2) beneficiaries at 25kg each. ‘The schedule of issuance shall be on a first come (with complete requirements) - first serve basis.
