I, NASRODIN A. LANGCO, Filipino, of legal age, married, and a resident of 3 rd Block

Santiago Subdivision, Bula, General Santos City, after having been sworn in accordance with
law, depose and say that:

1. I have applied for an Occupancy Permit with the Office of the Building Official of General
Santos City over a 2-Storey commercial building located at Magsaysay Avenue, General
Santos City, and have complied with all the requirements;

2. For the assurance of the tenants of the said commercial building, I hereby undertake to
follow-up the processing, issuance and release of the Occupancy Permit with the Office of
the Building Official of General Santos City in order that the same shall be issued on or
before January 31, 2015;

3. I am executing this affidavit to attest to the truthfulness of the foregoing and for whatever
legal purpose this may serve.

St. cor. Santiago Boulevard, Dadiangas South, General Santos City, after having been sworn in
accordance with law, depose and say that:

1. I am the tenant of a one (1) unit in a commercial building located at Cagampang Street cor.
Santiago Boulevard, Dadiangas South, General Santos City wherein I conduct my pawnshop
business under the name and style “P-Mart Pawnshop”;

2. I hereby undertake to apply, process, facilitate, and regularly follow-up for the issuance by
the Office of the Building Official of General Santos City of the following:

a. Renovation Permit
b. Occupancy Permit
c. Electronics Permit, and
d. Ramp for Persons with Disability (PWD).

3. I undertake the complete the above on or before March 31, 2015;

4. I am executing this affidavit to attest to the truthfulness of the foregoing and for whatever
legal purpose this may serve.
