Dear Dr. Gabriel,

This is to formalize our proposal for Implementation of Centralized Patient Registration in our
Hospital Information System.

Why are we changing to a centralized patient registration?


 This will avoid the double entries in patient registry.
 Centralized registration will make patient’s checking-in immediate, right from the hospital
reception.
 Department staff will have more time to assist patients.
 A standardized process across all departments for higher levels of efficiency.
 Having one place for patient registration will decrease confusion of where to go and help
navigate our patients through the building.

How will Centralized Registration Work?


 Centralized Registration will be conveniently located on the hospital lobby besides the
Information area, so that as soon as the patient walk into the building, they can check in
and have them registered in the hospital system and receive instructions on where to go
next.
 Other departments are no longer required to register the patients.
 If the patient proceeds to the concerned department without them being registered in the
system, our personnel will instruct them to proceed at the Information area for them to be
registered before our healthcare team assist them.

What do patients do once they have been registered?


 Once they are already registered in the Hospital Information System, they will be instructed
to proceed to the department of their appointment or concern, where our healthcare team
will be assisting them.

REQUIREMENTS:

1. Manpower

 This process will require at least two to three (2-3) personnel who will be in-charge of
processing patient’s registration upon check-in in our hospital.
 Centralized Registration will be 24/7 so there must be at least one personnel on duty per
shift.
 For 10pm-6am shift, we can assign Information and Admitting Staff on duty to register
patient since outpatients are infrequent during that time.

2. Hardware and Network

 Workstation requires an established network for accessing Hospital Information System.


 This will require one (1) unit of Desktop Computer on which the system will be installed.
 One (1) unit of printer for printing of forms and Patient ID.
