AFFIDAVIT OF VIDEOGRAPHER

I, PO1 ELORDE A LAZO JR of legal age, single and an active member of the
Philippine National Police presently assigned at the as Intelligence Operative, after
first being sworn do hereby depose and state:

1) That, relative to the arrest of BRYAN URSUA y CARINO by virtue of warrant

of arrest with CC No.231119 at PUROK 2 BAGONG KALSADA,
PALANGINAN, IBA, ZAMBALES I was designated as videographer tasked to
record the said operation;

2) That, I was using Android Samsung Galaxy J6+ with dual camera, 13 MP.
F1.9, 28mm (wide). AF, with CPU of Octa-core 1.6 GHz Cortex-A53, as
Alternative Recording Device since our office has not been issued yet with
body-worn cameras. Its battery features include Li-Ion 3000 mAh, non-
removable with talk time up to 21 hours and music play up to 76 hours. Before
the operation, the said alternative recording device was fully charged;

3) That, I started recording the operation when we arrived at the place where the
suspect arrested. covering the arrest by virtue of the above cited warrant until
he was brought to Police Station for documentation and proper disposition.

4) That, I downloaded the said video to a USB for submission as part of our
evidence for the above- said operation and had it sealed in plastic cellophane
and marked it with CBD-2023. Except for the undersigned, nobody else had a
hand or custody of the said device:

5) That, I am executing this affidavit to attest to the truth of the foregoing and
pursuant to AM No.21-06-08 -SC (Rules on Body-Worn Cameras in the
Execution of Warrants).
