What Happened?

• A contractor's employee suffered a head injury when he fell from height through a false ceiling whilst
01. removing a HVAC duct. While attempting to unfasten the duct retaining bolts, the IP stepped on the
false ceiling below which was not able to support the IP’s weight and subsequently gave way.

Lost Time Incident


Why did it Happen?
The incident is under investigation however early indications are:
Early Incident Notification

• The approved methodology for duct removal was unsuitable due to restrictions in the location of
existing MEP services.
• Insufficient warning signs or barriers were in place to alert operatives about the risk of accessing
the restricted area/fragile ceiling.
• Inadequate supervision and lack of coordination and communication between the subcontractor.
• Lack of work planning and notification to relevant persons.

What can you do?


• Ensure that each location has a specific, suitable, and sufficient method statement and risk
assessment. Any changes in the approved work methodology must be identified , recorded and
communicated.
• Implement a dynamic risk assessment to identify, communicate, and effectively mitigate any
risks.
• Establish a clear line of coordination and communication among all stakeholders.
• Identify and restrict access to all restricted/fragile areas using robust barriers and warning signs.
• Implement an effective work notification system in conjunction with the lookahead program.
• Provide adequate supervision to ensure that work is being carried safely.
• All projects to conduct a stand down (WSP people) to highlight the incident and emphasize the
importance of Safety Shield in identifying and mitigating risks.
