    On April 27th, 2022 at an EPC 1 project site, the hook from a hand operated chain hoist dislodged while lifting a load in the U310-F201 area, striking an operator and causing a slight shoulder injury. The incident occurred due to improper work assessment, negligence in rigging technique by using the chain hoist horizontally instead of vertically, and applying excess load. To prevent future incidents, competent supervision is needed during lifting activities, shortcuts avoided, using the right tool for the job, and improving risk perception of rigging techniques through stand down meetings.

    What happened ?

    On 27 April 2022, at around 11:30 AM at Area-


    U310-F201 loaded hand operated chain hoist
    hook dislodged from the structure resulting hook
    slipping back approximately 6m and striking Hook dislodged
    operator who was assisting in the activity.
    Operator got slight injury on back side of
    shoulder blade left side.
