ANTERIOR SHOULDER (GH) INSTABILITY

✔ Anterior (Apprehension) Release Test (“Surprise”)

 POSITION: Supine with shoulder abducted between 80 - 120˚, forward flexed up to 20˚ and
laterally rotated 30˚
 EXAMINER: Stabilize elbow and distal humerus ; grasp humeral head and lift it forward
 Examiner draws the humerus forward
 (+) SIGN: reproduction of symptoms INDICATION: Anterior Instability
 (+) SIGN: hears a clunk (click) INDICATION: Anterior Labral Tear
 Test modification of the Load and Shift test

✔ Crank (Apprehension) and Relocation Test

 CRANK TEST
o POSITION: Supine
o EXAMINER: abducts the arm to 90˚ and laterally rotates the patient’s shoulder slowly.
By placing the hand under the GH joint to act as a fulcrum
o (+) SIGN: Pain and apprehension INDICATION: Traumatic Instability
 RELOCATION TEST
o If patient experience apprehension during ER, examiner places firm hand over the
anterior shoulder, increasing shoulder stability
o Examiner again applies an ER to the shoulder
o Apply a mild anterior-directed force to the posterior humeral head when in the test
position to see if apprehension or pain increases
o (+) SIGN: Apprehension during 1st ER INDICATION: Anterior instability of GH Joint

❗ Load and Shift Test

 POSITION: sits with no back support

 (+) SIGN:
o Normal Laxity: 1 – 25%
o Grade 1: Head rides over the glenoid rim (25 – 50%)
o Grade 2: head over rides the rim but reduces (> 50%)
o Grade 3: head over riding the rim and remains dislocated
