ABSTRACT

 250-300 words only, italicized

 Discuss the major aspects of the entire paper which includes:
- the overall purpose of the study and the research problem(s) you investigated;
- the research design and methods of the study;
- major findings or trends found as a result of your analysis; and
- conclusions and major recommendations
 Provide 5-10 key words.
I. CONTEXT AND RATIONALE

Points and Sequence of Discussion:

1st – Provide the general description, global, national or local trend related to the
research topic. This should be interesting and specific.
2nd – Cite school or division data, information, observation and the gaps and issues
pertaining to the research topic.
3rd – Introduce briefly your proposed innovation/intervention/strategy and back it up with
DepEd policies and/or up-to-date related literature
4th – provide the purpose of the study using the school data and the identified gaps and
issues as the primary reason why you are conducting the action research
5th – provide the significance of your study or its contribution to the teaching and
learning or to the body of knowledge

Make sure to have smooth transition between paragraphs

II. ACTION RESEARCH QUESTIONS

There should be a question on the effect or effectiveness of the proposed innovation/
intervention/strategy

III. INNOVATION/INTERVENTION/STRATEGY

Points and Sequence of Discussion:

 Provide definition/description of the intervention.
 Include the rationale, objectives, and the extent or limitation of the intervention.
 Describe how this intervention addressed the problem or issue.
 Provide a clear procedure on how you implement the intervention and what was the
specific sequence of actions.

IV. ACTION RESEARCH METHODS

A. PARTICIPANTS AND/OR OTHER SOURCES OF DATA AND

INFORMATION
 Discuss who the respondents/participants and how they were chosen (sampling
method) for the study.
 Provide the total number of participants/respondents and their profile or conditions if
there is any.

B. DATA GATHERING METHODS

 Discuss the research design employed and why it is the most appropriate.
 Discuss the research tool/instruments used
- Definition (purpose of the instrument – information it collects)
- Inclusions (number of items, coverage, type (survey questionnaire, Likert scale,
etc.)
- Validity and reliability
 Ethical considerations involved in the process

C. DATA ANALYSIS
 Discuss the statistical method/treatment used to analyze the data and why it is the
most appropriate.
V. DISCUSSION OF RESULTS AND REFLECTION

 Refer to the research questions. Use subheadings (bold and italicized) which are
based on your research questions for the presentation of the results.
 Use table, graphs or figures to present the results (text first before table, graphs and
figures)
 Assess the meaning of the results of the study by evaluating and interpreting the
results. Do not just discuss what is in the table, graph, and figure.
 Ponder on what you learned from the study, its implications, knowledge contribution
and future research recommendation.
VI. ACTION PLAN FOR DISSEMINATION AND UTILIZATION
 Provide a detailed explanation of how the outputs and recommendations of your
study will be utilized (LAC, INSET, Research Conference, etc.)

VII. REFERENCES

 Include all references using APA (American Psychological Association) Style for in-
text and reference list citations

VIII. APPENDICES
Attach the following documents:
 Memorandum and other pertinent documents relative to the conduct of the study
 Copies of research tool/instruments used (blank)
 Copy of the informed consent (blank)
Photo documentation of the activities
