Dear Aisha:

On behalf of CarePartners, we are pleased to offer you with this offer of employment as a Regular
Community Support Worker (CSW) with the Home Support Worker (HSW) classification within our
6WWL-Waterloo-Wellington Branch on the following terms:

1. Date of Commencement & Pre-Employment Requirements

o Upon review and approval of your Home Support Worker Certificate from an alternate
establishment, we are happy to offer you employment as a Community Support Worker -
HSW with CarePartners.

Your employment will commence on June 20, 2023. In order to receive assignments, the
below noted conditions of employment must be completed to CarePartners satisfaction.

o As a condition of employment, confirmation of current immunization history is


required. This includes:
o Current tuberculosis (TB) status must be received within 14 days of date of hire. TB
status requires either a 2-step Mantoux Test within the last 12 months or, if a
Mantoux skin test is contraindicated, a physician letter stating you are free of active
tuberculosis and safe to provide direct patient care.

o CarePartners requires staff to meet the Covid-19 Safety Protocols and Vaccine
Requirements for Health Care Organizations as outlined by the Ontario Government

and CarePartners infection control policies. All employees will be required to wear
appropriate PPE as directed by CarePartners and mandated by public health
authorities.

o As a condition of employment, new hire must demonstrate/attest proof of


receiving their full Covid-19 vaccination, which means having received the full series
of a COVID-19 vaccine or combination of COVID-19 vaccines approved by Health
Canada (e.g., two doses of a two-dose vaccine series, or one dose of a single-dose
vaccine series); and having received the final dose of the COVID-19 vaccine at least
14 days ago.

o This offer is conditional upon receiving a satisfactory Criminal Reference Check (CRC) and
Vulnerable Sector Screen (VSS):

o Criminal Reference Check:


By accepting this offer, you acknowledge and express your consent to submit a
Criminal Reference Check utilizing the Backcheck service arranged with
CarePartners. A satisfactory CRC must be received prior to your employment with
CarePartners commencing.

o Vulnerable Sector Screen*:


You will be required to provide an original Vulnerable Sector Screen or a receipt
from your local police force indicating that you have applied for the Vulnerable
Sector Screen prior to beginning your orientation.

Should a ‘positive’ criminal reference check or vulnerable sector screen be submitted,


CarePartners will consider the context of the nature of the offence(s), including the
circumstances surrounding the charges and convictions, and the specific duties of your
position. Two previous employment references from your most recent supervisor/manager
will also be required as part of this consideration. If the results conclude that you may
present a risk to the patients, CarePartners staff or the company, your employment offer
will be revoked.

*Any expenses incurred for meeting the above noted conditions of employment will be your
responsibility.

o Upon hire, you will be a member of Service Employees International Union (SEIU), Local 1
Canada, hereafter referred to as “the Union,” and your employment will be governed by
the terms and conditions of the applicable Collective Agreement.

2. Compensation & Benefits

o Your base rate of pay for direct service will be CA$19.91/Hourly. The indirect wage rate is
paid at $15.50 per hour. You will be required to attend orientation, specific meetings and
trainings and will be reimbursed at the indirect care wage rate for any mandatory

attendance. You will be paid bi-weekly by direct deposit, minus applicable deductions.

o You shall be entitled to two weeks’ vacation time and 4% vacation pay on wages earned as
defined under the Ontario, Employment Standards Act, 2000 per fiscal year (April 1 - March
31). You will accumulate vacation on a monthly basis, based on your active years of service
and number of regular working days per week.

o CarePartners shall pay a $500.00 Retention Incentive as follows:


o $250.00 shall be paid following the completion of 3 months of service.
o $250.00 shall be paid following the completion of 12 months of service.

Payment dates will be in line with our Payroll Schedule.

You must be employed by CarePartners at the anniversary dates to be eligible to receive the
corresponding retention incentive payment.

o You may have access to CarePartners Extended Group Health Benefits, if you meet all the
requirements for benefits in accordance with the company’s standard benefit package, as
amended from time to time. The company reserves the right to make amendments to the
above-mentioned benefit plans as required.

Should you meet the eligibility requirements (e.g., work a minimum of 1352 hours within
the qualifying period), you will be required to enroll in the Long-Term Disability, Life
Insurance and Accidental Death and Dismemberment benefits. You do not have a choice to
waive Long Term, Disability, Life Insurance and Accidental and Death Insurance benefits if
you meet the eligibility requirement, this coverage is mandatory. CarePartners monitors
eligibility requirements. If you meet the requirement, you will be automatically
enrolled. You are responsible for reading the Group Health Benefits Policy/Guideline upon
hire, a signing acknowledgement is required to ensure you understand your entitlements
and obligations.

If an eligible employee does not require health benefits due to coverage under a spouse's
group insurance plan, the employee will be required to complete the forfeiture section on
the benefit form. Evidence of additional coverage is required in order to opt out of the
health benefits.

o Employees who use their personal vehicle for business use may be reimbursed for this
usage. The rate per kilometer for travel expenses is currently $0.45/km. It is your
responsibility to inform your insurance provider if you have a vehicle that you use for work
purposes (i.e., travelling to Patients).

It is strongly recommended that employees driving their own vehicles on company business
carry a minimum of $2,000,000 automobile liability insurance. The vehicle must be
maintained in a safe operating condition and covered by adequate insurance for business
use. Employees are solely responsible for the upkeep and maintenance of this
transportation and for any costs associated with the operation thereof, including the cost of

any parking tickets, fines for driving infractions and/or any liabilities arising from the
operation of the vehicle.

3. Working Hours

o Your schedule will be days, afternoons/evenings and weekends - flexible hours, although
this schedule may vary from time to time. You agree that scheduled assignments may
include evenings and weekends. You will be required to complete a ‘Scheduling Form’ to
aid us in offering you work. Hours of work are variable and the hours of work per week are
not guaranteed.

o Working schedules must reflect a high degree of flexibility; the need of the patient, the
Home and Community Support Services (HCCSS) and CarePartners may revise, amend or
extend your workdays or work hours should the need arise. CarePartners reserves the right
to alter your work schedule to ensure compliance with patient requirements.

4. Duties, Responsibilities & Obligations

o During your employment, you will report to a Personal Support Manager or such other
Supervisor as the company may direct. You will be expected to carry out the duties
assigned to you in competent and efficient fashion. All reasonable accommodation will be
provided for this position. If you have any questions or concerns, you will be expected to
bring them to your Manager or designate. The current duties and responsibilities for your
position are set out in your job description and may be amended from time to time.

o You agree that you will maintain all registration/certification/qualification requirements,


which are applicable to your position, at a standard that is satisfactory to your position at
CarePartners.

o You may receive company property for your related position. You are responsible for all
company property in your possession. You will be responsible to replace any damaged or
lost items at the current cost as deemed by the company.

o This offer of employment, in conjunction with the applicable Collective Agreement,


represents the entire agreement and supersedes any previous agreements, oral or in
writing, between you and CarePartners.

o You agree to comply with all company policy, practices, rules and regulations, and
instructions during your employment, and as may be amended from time to time. All
employment rights and obligations are in accordance with employment laws of Ontario,
and guided by company policy and/or provisions of the Collective Agreement with the
Union.

5. Termination

o Upon the termination of our employment relationship, you shall return to CarePartners all
documents, records, items and materials in your possession or custody belonging to

CarePartners or its patients and you shall not retain any copies (i.e., electronic or soft
copies) thereof.

o Your initial six (6) month period will be probationary. Either party may terminate the
employment relationship at any time during the probationary period without advance
notice or justifiable reason.
