Dear Madam:

Learning, thought creativity, and intelligence don’t just come from the brain alone, but from the entire body. Movement
combinations increase memory, order, and sequencing skills. Creating dances and engaging into arts also increases self-
esteem which is so very important to our students learning. By engaging to these activities, students would be able to
value the essence of being physically, socially active and will be able to unleash their potentials.

Hence, we would like to conduct a culminating activity in Health-Optimizing Physical Education 4 in collaboration with
Contemporary Philippine Arts from the Region dubbed as “Healthy Body and Mind, Key to Unleash Potentials: Senior
High Datu and Dayang 2024”. It will be held on January 15, 2024 from 1:00 to 5:00 in the afternoon at the Basilan
National High School Gymnasium. This activity will serve as their final output/performance task for this semester as
stipulated in the Most Essential Learning Competencies (MELCs) in H.O.P.E. “Organizes fitness event for a target health
issue or concern” (PEH12FH-IIo-t-17) and in C.P.A.R.” Create the intended final product using appropriate materials for
the best possible output’’(CAR11/12ap-0f-h-17).

With this, we would like to respectfully seek for your approval to allow us to conduct the mentioned activity, to use the
gymnasium, and sound system for this purpose. Also, we are requesting if we can have the shortened class period of 40
minutes per subject before the culminating activity. Attached here is the propose schedule for the shorten period of
classes.

3:00-4:00 pm 11:40 – 12:20 noon
