GUIDELINES 4. Procedure
S.Y. 2022-2023
1. Qualifications for Iskolarealeño Completely fill out the Iskolarealeño Application Form
a. Must be a Filipino citizen, regardless of sex, civil status, religious affiliation or belief.
b. Must be a bonafide resident of the municipality of Real, Quezon and must submit any of the
following proof: Attach clear scanned copy of Form 138 or
b.1. Voter’s registration
High School Report Card
b.2. Birth Certificate from the Local Civil Registrar of this Municipality indicating Real,
Quezon as Birth Place
b.3. Income Tax Return of parents
b.4. Certification from MPDO that the applicant is included in Community Based Attach Certificate of passsing the entrance
Monitoring System (CBMS) database test in school of choice (if available)
b.5. Postal ID
b.6. Certification from Pantawid Pamilyang Pilipino Program (4P’s) representative
c. Must have passed the Iskolarealeño qualifying examinations; Attach Brgy. Certificate of Indigency and 1 proof of bonafide
d. Must have passed the entrance examination of the chosen State Universities and Colleges, residency cited in the Qualifications (b1-6)
Private Local Universities and Colleges and State-Run Technical Vocational Institutions;
e. A graduate from Secondary level with a Final Rating of at least 90%.
