RESEARCH AND FIELWORK ACCOMPLISHMENT FORM

Instruction:
1. Fill-out this form with all the necessary detailed information.
2. Choose the type of activity and provide the information.
3. Upload this form thru the link provided in the Blackboard.
4. Submission shall be done every Friday of the week.
5. Attached photos, videos, and other documents to support the reported activities thru a Google Drive Link. The group must create a
group Google Drive link send only the link, not the files, videos, or any other documents. (Note: Failure to submit proof/evidences for
research work and or fieldwork activities, NO GRADE or ZERO will be given for the Accomplishment Report.
6. The Weekly Group Accomplishment Report as submitted will the basis of grading for Case Study and the Final Case Study.

Notes:
1. For INDIVIDUAL TASK ASSIGNMENT (Research or Fieldwork), indicate the name of the members and the assigned research task
and or fieldwork assignment).
2. Group Leader must accomplish FIELDWORK ATTENDANCE SHEET and upload as part of the Google Drive link.

1. This requirement shall be submitted in the BB under Message Section every Monday of the Week by the Team Leader.
2. All members of the group shall submit proof or evidence of accomplish work stated in the assigned task/assignment to be place in
Google Drive (create folders per members of the group to organize the files) and send the link to the Class Adviser. The Team
Leader shall be responsible for uploading this requirement.
