RPGA Infraction Report Form

Infractions of the RPGA General Rules must be
entered into the RPGA Penalty Database and become
a part of a player's permanent record. The Senior GM
for and event must complete this form and return it
with any other relevant information pertaining to the
infraction. If you feel that the infraction warrants
further RPGA investigation and/or penalties, please
contact the RPGA policy administrator at
rpgagm@wizards.com as soon as possible. Please
provide a separate form for every individual involved
in an infraction. Please include interview notes in the
section provided or attach any relevant documents to
the form if you need more space. Please make your
notes as clear as possible to aid in our processing. We
highly recommend keeping a copy of the completed
form for your records.
