JOINT AFFIDAVIT OF UNDERTAKING

I Nico M Ordovez, of legal age, Filipino, married and a resident of Brgy. Dumarag,
Pastrana Leyte,after having been duly sworn to in accordance with law , do hereby deposes and
states;

1. That I am a registered Criminologist whoses PRC issued ID expired last

December 3,2021;

2. That upon transaction at PRC for the issuance of new certificate of Registration,
we were advised that we have to submit a new and unexpired Certificate of Registration;

3. However, pending issuance of the new Certificate of Registration we are

submitting this affidavit of Undertaking to undertake that we will immediately submit to the
Professional Criminologist Association of the Philippines (PCAP) as soon as PRC issues me a
new Certificate of Registration;

4. That I execute this affidavit of undertaking voluntarily and to comply with the
requirements of the PCAP with respect to the issuance of Certificate of Registration

IN WITNESS WHEREOF, I have hereunto affixed my signature this 30th day of January
