I, PRAXEDES M. REAZO, of legal age, Filipino and resident of

Alabat, Quezon, after having been duly sworn to in accordance with law
hereby depose and state:

That I know my true correct spelling of name is

PRAXEDES MASCARIÑA-REAZO, I been using in my
identification card and any legal documents;

That however, I found out in Tax Declaration TD No. 02-

0005-00306 my name appeared as PRAXIDES MASCARIÑA;

That I hereby attest that the PRAXEDES MASCARIÑA-

REAZO AND PRAXIDES MASCARIÑA refer are but one and
the same person;

That I am executing this affidavit to attest to the truth of the

foregoing facts and/or for whatever legal purposes this may serve;
