· MAR (medication administration record)
· Sterile Tuberculin Syringe and needle (or
needleless adapter)
· Ampule or vial of medication
· Filter needle, if required
· Alcohol swab
· Gauze squares, if opening an ampule

Intradermal
1. Check the expiration date on the medication
vial/ampule
2. Assess the integrity of our needle instrument, selecting
1.0 ml tuberculin syringe
3. When selecting the injection site, inspect the skin for
bruises, inflammation, edema, masses, tenderness, or
any signs of previous injections.
4. Forearm site should be 3 to 4 finger widths below the
antecubital space and 1 hand width above wrist or
inner aspect of the arm.
5. Assist the client in a comfortable position and clean the
site with alcohol pad and remove the cap from the needle
6. With a non-dominant hand stretch the skin over the
site using the forefinger and thumb, inject the needle
slowly at 15- degree angle, bevel up, advancing no
more than 1/8 of an inch under the skin.
7. Slowly inject the medication and notice the small bleb
formed under the skin surface.
8. Withdraw the needle and apply gentle pressure with
alcohol pad. Do not massage injection site.
9. Discard uncapped needle and syringe into sharps
container. Do not recap used needle. Removed gloves
and wash hands.
10. Follow ENDDD steps
Ensure comfort & safety
Note questions or concerns from client.
Dispose materials properly
Disinfect the area and your hands
Document and report the procedure and findings
