Objectives:

 To identify the needs of client and assess his environment.

Presented Problem:
The client expressed that he is in dire need of financial support as he was struggling
with his painful arthritis which made him walk barely.
Client-Worker Interactions: (Describe how did the client respond to the interventions that
you provided?)
The client who is suffering from his condition “arthritis” which he called it, has been
thankful to the Social Work intern for coming. The whole interactions revolves around
the interview and the intern obtained partially of his need and background.

Student’s feeling and Observation:

The social work intern was overwhelmed with the client situation as he barely get
support from his family relatives. That’s why, he sought help from the Provincial Social
Welfare and Development Office (PSWDO) for some help.
Learning’s and Insights:
The Social work intern learned how crucial is to pay a home visit to clients because
some of them are not able to come in agency regularly because of their conditions.
Plans for the Next Meeting:
The intern will seek consultation from his AFI regarding the conditions of her client
and what or how will she able to give him the best interventions knowing the workplace
she was assigned to.
