CIP Minutes of the Meeting

With this quotation, we are inspired to launch the Continuous Improvement Program (S.Y. 2020-2021) On
February 16, 2021, we held a meeting at Dela Paz Main Elementary School with the following objectives:

 Planning and prepare of the new Continuous Improvement Program

 Compose a team and CI members
 Designation of work tasks
 Prepare the survey to be used in CI project

The CI meeting started with the opening remarks from our School Head, Mr. Jonar C. Olicia followed by the
orientation of the former CI coordinator, Mrs. Flory M. Miguel. She discussed the first step of the CIP which is
Team Composition. We came up with 2 teams composed of 7 members each. These are the list of 2 teams
together with their designated task.

The CI Team held a meeting on March 11, 2021, 09:00 in the morning. The two leaders, Mariz Nayle and
Leandro A. Bambe distributed the roles and responsibility of each member on what to do in our survey. They
discussed what’s inside the survey in case there will be questions ask by the parents, students and the teachers.

After the meeting, assigned members started printing all the survey forms.

CIP members held an online meeting on May 17, 2021 (Monday). Ms. Mariz Nayle, Sir Leandro Bambe
and Ms. Flory Miguel explained and discussed the affinity diagram and Why-Why Diagram.

On May 19, 2021 (Wednesday), members of the CIP team had a meeting with the school head Sir Jonar
