I, PABLO C. REAMBONANZA, of legal age, Filipino citizen, married and a resident of

Pautao, Bacuag, Surigao del Norte after having been sworn to oath in accordance with law hereby
avers the following to wit:
1. That I am the person mentioned above who seeks to correct the entry of my name in my
Marriage Contract from BASILIO RAMBONANZA to PABLO CASCARA
REAMBONANZA as correctly stated in my Certificate of Live Birth;
2. That one of the requirements in the correction is the earliest school record or earliest school
document;
3. That I am a graduate of Payapag Elementary School in Bacuag, Surigao del Norte;
4. Unfortunately, I cannot provide such a document because the records are already damaged
due to long period of time and is already beyond recovery;
5. That I attest to the truthfulness and veracity of the foregoing facts and statements in support
of the preceding purpose and for all other legal intents and purpose/s it may serve.
