AFFIDAVIT OF NO MARRIAGE

I, EVELYN ABRAHAN ACABO, of legal age, single, Filipino Citizen and a

resident of Pautao, Bacuag, Surigao del Norte, after having been duly sworn to in
accordance with law, do hereby depose and say;

That I am the same and identical person who caused the execution of this
instrument;

That I vow and affirm that up to this date, I am still SINGLE and did not contract
any marriage to anybody here in the Philippines or to any country for that matter.

I am the biological mother of ALVIN ACABO, who is born out of wedlock on

April 12, 2000 at Pautao, Bacuag, Surigao del Norte;

That I am executing this instrument to apprise and inform concerned government

agency of the foregoing facts and attest to the truthfulness and veracity of the same.
