    Case: 1.

    Deceased fully-paid CA awardee prior to loan individualization

    2. Deceased CA awardee, included in the loan individualization

    Advise (solicited from the Legal Department):

    1. Surviving heir/s shall request the CA for transfer of rights with the ff documents:

    > notarized extra judicial settlement with waiver of rights if applicable

    > certification of publication in a newspaper of general circulation of the extra judicial

    settlement

    > copy of death certificate

    2. CA shall approve the request thru a Board Resolution.

    3. CA shall endorse the approved request to NHA for substitution of awardee/s in the masterlist
    together with the supporting documents.

    4. PO ComRel shall undertake substitution after PM’s approval; ComRel shall update subsidiary LIS/
    provide back-up diskette to EMD.

    5. For fully-paid awardee/s prior to loan individualization, deed of sale will already be processed in the
    name of the approved transferee/s.
