5. Along with the filled-up Application Forms the attested photo copies of the documents required:

a. Xerox copy of Panchayat / Municipality/ state Hospital / Nursing l-lome Birth Certificate to be enclosed.
The Xerox copy must be attested by a component authority.

b. Christian candidates must submit a copy of Baptism Certificate duly attested by the Presbyter-in-
charge/ Parish Priest, along with a bonafide membership certificate from the church'

c. The Parents/ Guardians, who draw Salary will submit the Form-16 or any equivalent document from
the Employer, are to be attached. Those who are self-employed or who have their own business must
submit a personal statement of their yearly income along with a Xerox copy of INCOME TAX RETURN.

d. Caste Certificate if any

e. Documents relating to last School attended for the Candidates seeking admission for Class -ll (Two)
onwards, such as Pass Certificate/Mark Sheet /Report Card showing promotion, Original Transfer

3. Enter relevant credentials: Student Application No.
4. Follow subsequent steps to select your Class of Admission {Xl) and then pay the requisite
School Fees Online: Monthly Fees of April 2023 along with Session Charges and New
Admission Fees through the DebitlCredit CardlUPUNet Banking options only.
5. Generate the Fees Payment Challan.
6. Download and Print the Undertaking Form from the School Website Notice Board. Carefully
read the Terms and Conditions before putting in your Signatures and Contact Numbers
