This is to certify that (NAME OF THE OFFICIAL), of Barangay (BARANGAY), Jasaan,
Misamis Oriental has been elected to the position of POSITION and has complied
with the following requirements, to wit:

1. Proclaim by the Barangay Board of Canvassers (BBOC) last (DATE

PROCLAIMED) Barangay and SK Elections;
2. Submitted a Certificate of Compliance; and
3. Taken an oath and submitted a copy of Oath of Office.

It is certified further that the abovementioned has assumed in the office effective
as of date of issuance of this certificate based on DILG Memorandum Circular (MC)
No. 2023-116 dated October 31, 2023.

For all intents and purposes derived from the above compliance, it is hereby
certified that the abovementioned has fully acquired all the authority to exercise its
functions.
