AFFIDAVIT OF UNDERTAKING

I, Rachel N. Yu , of legal age, single/married, Filipino, and a resident of

Gredu , Panabo City, Davao del Norte , Philippines, after having been duly sworn in accordance with law, hereby
depose and say:

1. That I am the parent/guardian of Gabriel Jeush N. Yu , (age) 16 years old, from

Panabo City National High School , one of the official participants for
THAILAND INTERNATIONA MATHEMATICAL OLYMPIAD (TIMO) 2023-2024 FINAL ROUND;
2. That I will allow coach/Mr./Mrs./Ms. E l l e n J o y J . S o m b i l o n to accompany and travel with my
son (son/daughter) in the said event;
3. That they will attend the THAILAND INTERNATIONA MATHEMATICAL OLYMPIAD (TIMO) 2023-2024 FINAL
ROUND at Bangkok, Thailand this coming April 26-29, 2024;
4. That we will avail the TIMO Registration for my child’s slot.
5. That we agree to allow the THAILAND INTERNATIONA MATHEMATICAL OLYMPIAD (TIMO) to post the results of
our children online;
6. That we will book our own accommodation and travel package on our own account and risk;
7. That I am fully aware in availing this option, Math Olympiads Training League Inc. (MOTLI) are not responsible
from any untoward incidents, injuries and damages incurred during the duration of the said travel;
8. That we willingly and voluntarily release Math Olympiads Training League Inc. (MOTLI) free from all risk and
responsibility from any actions, liabilities, damages that may arise during the duration of our travel from April
26-29, 2024;
9. That we will avail travel insurance during the duration of our travel for THAILAND INTERNATIONA
MATHEMATICAL OLYMPIAD (TIMO) 2023-2024 FINAL ROUND;
10. By signing this agreement, both parties will not be liable for failure to carry out the agreed arrangements which
are caused by uncontrollable circumstances such as but not limited to acts of terrorism or similar acts of labor
disputes, strikes, civil disorder, war, or other similar cause of threat, acts of God, outbreaks of disease or
epidemics/pandemics in the country where the TIMO event is located, government regulations, travel advisory,
fire, disaster, damage or any destruction or any other condition beyond our control.
11. That I am executing this Affidavit of Undertaking to assume full responsibility and obligation being the parent
of one of the TIMO Final Round delegates 2023-2024; to attest the truth of the foregoing statements and for
whatever legal purpose this may serve best.
12. That the registration fee is non-refundable.
