After the Test Administration
1. Checking the test items were conducted based on
the prescribed period.
2. The teachers utilized the results of test in
preparing Intervention Materials/ Test Bank.
3. Test results analysis were conducted by:

4. The school developed a feedback mechanism on
the results of the test.

Sir:

In order to carry-out the Vision, Mission and Core Values as well as the goals under the Matatag
Agenda of DepEd, the project SMILE of Deped Region V and THE VILLAGE INC. of this division,
the undersigned would like to ask permission to hold a 1-day meeting/conference of all the School
Heads of this district. This will be on January 15, 2004 at Matnog Central School, Matnog, Sorsogon.
The following agenda will be discussed, to wit;
1. INSET
2. Feedback of my visit to 20 elementary & secondary schools of this district
3. Monitoring Tool on the conduct of Quarterly Examination (designed by the undersigned, as
attached for perusal)
4. Quarterly Instructional and Supervisory Plan (alignment of District and School Heads’ Plan
5. The Special Education Fund (SEF) Budget for CY 2024 each school will prepare Program of
Works/Bill of Materials(approved during January 9, 2024 Board Meeting)
6. Zonal Palaro (June 17-20, 2024, in Bulan, Sorsogon)
7. Other matter/s that may arise.

With regards to INSET, we would like to invite our Asst. Schools Division Superintendent (Mam
Bernie) to deliver an inspiring wisdom and thoughts during the Opening Program on January 24, 2024
to start at &:30 a.m at Matnog Central School, Matnog, Sorsogon.

I am hoping that the above-cited requests will be granted by your leadership as we work towards
attaining the Quality, Accessible, Relevant and Liberating Basic Education.
