AFFIDAVIT OF LOSS

I, PRINCESS IRA T. SANTILLAN , of legal age, Filipino, single, with

residence at Zone 6, Brgy San Isidro, Tarlac City, Tarlac after having been sworn to in
accordance with law hereby depose and say that:

1. I am a licensed professional teacher employed at Tarlac State University;

2. As such, I am a holder of a Professional Regulation Commissions (PRC) ID

issued by the Professional Regulations Commission, Manila;

3. Said PRC ID is always kept inside my wallet;

4. On 29TH February 2024, I was surprised to find out that my wallet was gone.

5. My wallet, containing my PRC ID, must have fallen from the pocket of my
jeans on my way home as when I reached our house and look for my wallet, the
same could no longer be found.

6. I took earnest efforts to look for my wallet and even went back to the streets
where I passed home and further went to my office where the same must have
fallen but to no avail, hence given up as lost;

7. I execute this affidavit to attest to the truthfulness and veracity of the foregoing
facts, and for the purpose of securing another PRC ID, and for whatever purpose
it may serve.

1. That I am a holder of the LANDBANK ATM card with

account number 0077194592 issued by Landbank
Tarlac City, Tarlac;

2. That sometime yesterday, when I arrived home from work

I discovered that my LANDBANK ATM card together with
my wallet was no longer in bag.

3. That despite diligent search and efforts to locate my ATM

Card and wallet, I could not find the same such that I
now believe that it is now lost beyond recovery;

4. Said ATM Card was not confiscated by any officer of

the law or the officers of LANDBANK by reason of any
violation of any law, ordinance, presidential decree or
rules and regulation of said bank; and neither was it
offered as collateral for any loan;

5. That I am executing this affidavit to attest to the truth of

the foregoing facts and to request for a replacement of
my LANDBANK ATM card.

Card and wallet, I could not find the same such that I
