LETTER OF INDEMNITY AND UNDERTAKING

name] [insert intake and year] [insert current semester] hereby fully understand and agree to the
following:

1. The purpose of the internship programme and participate in it on my own accord.

2. I shall be liable for and indemnify Sunway University (the institution and/or their appointed
staff) against any liabilities, loss, claims or proceedings, actions in respect of my mishaps,
injuries, including death of, and loss of or damage to property of, suffered by me or the
Organisation or third parties arising out of my negligence, any act and/or omission or
contributable to or was partly contributable to my performance of work that may occur in the
course of, or as a result of, my participation in the internship for any reason whatsoever.

3. I shall undertake full responsibility of my action and behaviour at all times during the course
of internship programme. The University shall not be liable for my misconduct or wrong
doing.

4. I am able to meet the financial commitments and expenses estimated to be incurred during the
internship period.

5. This letter of indemnity supersedes any other indemnity letter acknowledged between the
University and Organisation in which I will be undertaking my placement.

6. The University/School will support me throughout the internship placement and in helping
me address any issues faced during my tenure as an intern within the Organisation.
