I, Arnold C, Lasit, a student of Bulacan State University, Meneses Campus, Teacher

Education Department, do hereby agree and promise to abide by the rules of the University
during the limited face-to-face internship, On-Campus, inside/outside Bulacan State University
at Guiguinto National Vocational High School from February 2024 to May 2024, as well as
the pertinent rules and regulations of the facility/office/department/institute. I understand that
failure to abide with the rules and regulations would result to my discontinued participation in
the limited face-to-face internship/On-Campus Internship, inside/outside the Bulacan State
University.

I understand that during the limited face-to-face internship/On-Campus Internship,

inside/outside Bulacan State University, I shall be under the direct supervision of the faculty-in-
charge/adviser and the Department Chair/Institute Director. I promise to observe due diligence
in adhering to the DOH-IATF guidelines and protocols on COVID-19 mitigation and prevention.
I understand that I may be legally liable for violations of these protocols. I understand that if I
contract COVID-19 during the limited face-to-face internship/On-Campus Internship,
inside/outside the Bulacan State University, that damages shall be charged to my medical
insurance.
