AGREEMENT ON THE IMPLEMENTATION OF LIMITED FACE-TO-FACE CLASSES

This Agreement entered into and executed by and between:

Calamba Doctors’ College, with address at Virborough Subdivision, Parian, Calamba City, Laguna, Philippines,
represented by its Vice President for Academics, JOSE BACUD JR. , hereafter referred to as the SCHOOL;
and

Student XYZ, born on November 23, 2000, taking up Bachelor of Science in Nursing, with address at Phase 1 Block
4 Lot 3 Mabuhay Village, Cabuyao, Laguna Philippines, hereafter referred as the STUDENT,

and

Mrs. Julia XYZ, mother and legal guardian of the STUDENT, with address at Phase 1 Block 4 Lot 3 Mabuhay Village,
Cabuyao, Laguna Philippines, hereafter referred to as the PARENT,

WITNESSETH: That

Whereas, the SCHOOL, offers non-mandatory Limited Face-to-Face classes to enrich the degree program of
Bachelor of Science in Nursing in its commitment to provide quality education to its students and to respond to the
needs of the health care industry;

Whereas, the STUDENT, is a 4th year student who voluntarily enrolled in the Limited Face-to-Face Classes offered
by the SCHOOL for AY 2021-2022;

Whereas, the PARENT, is the legal guardian of the STUDENT who voluntarily gives consent to the STUDENT to
enroll in the Limited Face-to-Face Classes offered by the SCHOOL;

NOW, THEREFORE, for and in consideration of the foregoing premises, the parties acknowledge and agree to work
together as partners in the implementation of the Limited Face-to-Face Classes under the following terms and
conditions:

Responsibilities of the SCHOOL:


• Reopen their campuses for Limited Face-to-Face Classes subject to the approval of the concerned
CHEDROs of their applications to reopen;
• Formulate policies and guidelines on the implementation of the Limited Face-to-Face Classes;
• Monitor and evaluate the implementation of health and safety protocols and compliance with CHED-DOH
Joint Memorandum Circular;
• Continuously make improvements in their institutional policies, guidelines, and procedures and their
implementation;
• Submit to the concerned CHEDROs a weekly COVID-19 monitoring reports;
• The School retains the prerogative to deny student’s or any individual’s entry to school campus due to
probable health hazard to student or other individuals.
• The School reserves the right to implement screening and detection measures including but not limited
to, taking Student's temperature and inquiring about current symptoms, before allowing Student school
entry and participation to limited f2f classes.
• The School has the right to cancel or suspend classes if health and safety of students, and other
individuals may be placed at risk.
• Inform parents of their children’s health status in case of infection while in the campus.

Responsibilities of the STUDENT:


• Enter into an Agreement with Calamba Doctors’ College on the implementation of Limited Face-to-Face
Classes;
• Sign all the required documents necessary for his or her participation in the Limited Face-to-Face Classes,
including health declaration, this agreement etc.;
• Strictly adhere to the guidelines and policies of Calamba Doctors’ College in the implementation of
Limited Face-to-Face classes, including protocols to be followed while inside the campus, and
laboratories;
• Strictly follow health and safety protocols at all times;
• Student will comply with the one entry one exit policy.

• Student will not be allowed to leave campus to buy food. Thus, food packs must be prepared for students
to bring to school.
• Be transparent in declaring health conditions, including those of their family members;
• Present a NEGATIVE RT-PCR test result prior to attending Limited Face-to-Face Classes;
• Present a written permission from a licensed healthcare professional prior to attending Limited Face-to-
Face Classes in case STUDENT has any underlying medical condition;
• Not attend Limited Face-to-Face Classes when not feeling well, has COVID-19 symptoms or have been in
contact with anyone with a confirmed COVID-19 diagnosis in the last 14 days; and
• Comply with the provisions of this agreement.

Responsibilities of the PARENT AND OR GURDIAN:


• Make sure that their children are in good health condition before allowing them to go to school;
• Habitually remind their children to follow safety protocols and physical distancing at all times;
• Consistently remind their children to check the supply of their personal hygiene kit (i.e., alcohol/ sanitizer,
mask, face shield, gloves, tissue etc.) and bring it with them at all times;
• Prepare food and beverage for their children to bring to school;
• Be transparent in declaring their children’s health conditions, including those of family members;
• Do not allow their children to attend Limited Face-to-Face Classes when not feeling well, has COVID-19
symptoms or have been in contact with anyone with a confirmed COVID-19 diagnosis in the last 14 days;
• Sign the Parents Consent Form that they are allowing their children to attend Limited Face-to-Face classes
for laboratory courses;
• Comply with the provisions of this agreement.

Effectivity
This agreement shall take effect upon signing of all parties until the student has completed the semesters under
the Limited Face-to-Face Classes for AY 2021-2022.

IN WITNESS WHEREOF, the parties hereunto signed on this _____ day of ______________ 2021 at City of
Calamba, Laguna.
