_____________________________ to play for the 33rd Harbor Pilot Cup Invitational
Basketball Tournament 18under (Born 2005).

I acknowledge that his participation in this game involves risk, serious bodily injuries,
death, damage, and/or other harm that might result not only from a participant’s action,
inaction, or negligence but also from the action or negligence of others.

As the organizer, I understand that Harbor Pilot Sports has the right to make
tournament changes as necessary. I fully release and discharge HPS Organizers and its
employees, officers, and program sponsors from all liability on my son’s participation in the
said tournament.

I have read and voluntarily signed the permission form.
