hereby give my full consent and approval for my child to join and participate in:
Title of the Activity: R-SK Cup Basketball League 2023/Address of the Activity: Alfonso
Town Plaza Covered Court, Poblacion 2, Alfonso, Cavite.
I agree and understand that participation in basketball involves significant risks or death
associated with participation in community. These risks include the possibility of very
serious injuries which can occur for a variety of reasons.
Injuries and damages that may occur during or at any Basketball League activities are
fully undertaken by participants and parents.

I/We have read this participation, assumption of risk and agreement, and have signed it
freely and voluntarily without any inducement.
