SERVICE AGREEMENT

Temporary Worker/Reliever

Subdivision, Tinajeros, City of Malabon (hereinafter referred to as the "Company"), and
[Extra/Reliever's Name]

(hereinafter referred to as the "Extra/Reliever").

1. PURPOSE
The purpose of this agreement is to define the terms and conditions under which the
Extra/Reliever will provide services to the Company on an as-needed basis.

2. SCOPE OF WORK
The Extra/Reliever agrees to provide services to the Company as needed, including but not
limited to the tasks and duties given by the immediate supervisor.

3. COMPENSATION
The Extra/Reliever will be compensated at the rate of [Rate of Compensation] _______________ per day,
for 8 hours of work, excluding the 1-hour lunch break. Payment will be made on a weekly basis
every Friday.

4. SCHEDULE
The Extra/Reliever's schedule will be determined by the Company based on its operational
needs. The Extra/Reliever agrees to be available to work during the scheduled hours as
required by the Company.

5. RESPONSIBILITIES
The Extra/Reliever agrees to perform their duties diligently and efficiently and to comply with
all Company policies, procedures, and guidelines.

6. CONFIDENTIALITY
The Extra/Reliever agrees to maintain the confidentiality of any proprietary or sensitive
information they may have access to during the course of their employment with the Company.

7. TERMINATION
Either party may terminate this agreement at any time with or without cause by providing 3
days' notice to the other party.

8. ENTIRE AGREEMENT
This agreement constitutes the entire understanding between the Company and the
Extra/Reliever and supersedes all prior agreements and understandings, whether written or
oral, relating to the subject matter herein.

9. SIGNATURES
IN WITNESS WHEREOF, the parties hereto have executed this agreement as of the date first
above written.
