AFFIDAVIT OF DELAYED REGISTRATION OF BIRTH

I, DELFINO B. MALANA, of legal age, Filipino, single and presently residing at

Barangay Balug, Tumauini, Isabela, and after being duly sworn to in accordance with law,
depose and state:

1. That I am the applicant for the delayed registration of my birth;

2. That I was born in Barangay Balug, Tumauini, Isabela on December 25, 1965;

3. That I was delayed in registering my birth for the reason that my parents failed to
register my birth when I was born;

4. That it was only recently, when I applied for Philippine Statistics Authority
authenticated birth certificate, that I found out that my birth was not registered;

5. That I am executing this affidavit to attest to the truth and veracity of the foregoing
facts and in compliance with the requirements of the Local Civil Registry of the
Municipality of Tumauini.

That I am executing this Affidavit to attest to the truth of foregoing facts and for whatever
legal purpose it may serve.

IN WITNESS WHEREOF, WE have hereunto set our hands this 15th day of March,
