Description Design Certificate – Jumpform

1.0 Description & scope

Acrow Formwork & Scaffolding have undertaken the design and certification of the core jumpform system for the above
noted structure and project. The design has been documented on the following Acrow Formwork & Scaffolding design
drawings;

Refer Attachment A for drawing list.

2.0 Certification

We hereby certify that the designs described by the above noted drawings generally comply with the relevant provisions
of the following Australian Standards (subject to the items listed in Section 3.0);
 AS/NZS 1170.0:2002 Structural design actions Part 0: General Principles
 AS/NZS 1170.2:2021 Structural design actions Part 2: Wind Actions
 AS/NZS 1720.1-2010 Timber structures Part 1: Design methods
 AS 3610:1995 Formwork for concrete
 AS 3610.1-2018 Formwork for concrete Part 1: Specifications
 AS 4100:2020 Steel structures
 AS 6669:2016 Plywood - Formwork

3.0 Basis of certification

The design checks carried out have been undertaken using the technical data sheets, guides and manuals for the
proprietary items published by Acrow Formwork & Scaffolding.

Certification is subject to the following;

 Work must be carried out by suitable skilled and qualified trades in their respective areas;
 Drawings shall be read in conjunction with all project documentation and specifications. The client shall check
and be responsible for the correctness of all dimensions and any discrepancy shall be reported immediately.
Dimensions shall not be obtained by scaling the drawings;
 All proprietary products nominated on the drawings shall be installed and operated in accordance with the
manufacturer’s specifications. All proprietary items to be in good working order;
 All formwork and falsework shall be safely erected in accordance with the Formwork Code of Practice,
manufacturer guidelines and workplace health and safety legislation;

 All formwork shall be inspected by a competent person/engineer in accordance with the relevant Formwork
Code of Practice. Variation in design may be permitted in isolated areas providing design criteria is achieved
and verified by an engineer;
 It is not possible to convey every single piece of information on any formwork drawing. It is the responsibility
of trained and competent formworker’s to read the design drawings, relevant manufacturer technical manuals
and use their own expertise to fill any gaps in information. If in doubt – ask;
 The formwork shall be monitored prior to and during the concrete placement and adjusted as necessary.
Additional bracing or anchoring shall be installed in deemed necessary;
 Maximum wind speed during the jumping process & operation = 16m/s (57.6 km/hr);
 Stacked materials shall be removed & shutters shall be secured to the jump during the jumping process;
 No point loading on the ply allowed in between joist location (ie. Install dunnage over top of joists);
 Monitor loadings to ensure they do not exceed design loadings. Platforms shall be evenly loaded;
 Concrete shall be placed in even layers within vertical formwork not exceeding 400mm thickness;
 Refer to drawings for specific arrangement and design notes;
 Note that this certification is for the structural capacity only – it does not check dimensions or set-out accuracy;
and
 This certification is for the nominated structure and project only.

The jumpform design allows for the installation of the concrete placing boom based on the details and loads provided
by the Contractor’s consulting engineer. Acrow have relied upon these loads and have not undertaken independent
verification. The concrete placing boom loads were provided on the following drawings by O’Hearn Consulting;

4.0 Conditions of certification

It is the responsibility of others to certify the following elements;

 Permanent structure capacity to resist any construction loads; and
 Temporary structures required for erection & dismantling.

This certificate is limited to the compliance of the structural integrity for the above-mentioned items only. Acrow
Formwork & Scaffolding accepts no responsibility for information that has not been expressly identified as part of this
advice. This certificate can only be relied upon by the addressee and cannot be used or adopted by any third -party.
This certificate does not relieve the contractor of their obligations to comply with all relevant standards, codes of
practice and statutory requirements.

Whilst this certificate addresses the structural capacity of the above arrangement in its final erected state, it does not
relieve the contractor or any other party of their responsibility of ensuring that all equipment used is in a good state of
repair in accordance with the manufacturer’s specification, meets the minimum requirements of the current code and
the design notes as well as any local regulations for its intended use. Safe work method statements for installation of
the formwork are the responsibility of the contractor.

This certificate shall not be construed as relieving any other party of their responsibilities, liabilities, or contractual
obligations, and does not constitute an inspection certificate.
