1. Health Perception / Health Problem Pattern


- The patient mentioned that she is mostly feeling good because her medication stabilizes her . She
neither smokes nor drinks recently as she is now taking medications.She maintains her independence when it
comes to grooming and maintaining good hygiene.

2. Nutritional – Metabolic Pattern


- She follows the scheduled meals for them. She obediently takes her medicine and keeps herself
hydrated. The client is not picky with food as she mentions that she likes all kinds, especially sweet foods,
chocolates and beverages. Her food allergies are seafood, grated coconut, and mayo.There are no issues with
eating.Observes gusto in eating and finishes her entire prepared meal. She is not wearing dentures.

3. Elimination Pattern
- Patient often doesn’t feel discomfort during bowel elimination. She regularly urinates. The patient
perspires and has no issues with odor. She also does not use any assistive excretory device, unusual
discharge was noted on the client.

4. Activity – Exercise Pattern


- Patient enjoys a selection of music although she doesn’t dance. Observed the client singing going
along with lyrics while listening to music while slightly nodding her head. She independently performs most of
her hygiene activities. She starts her morning routine initially and then goes for a walk whenever she feels like
it.

5. Sleep – Rest Pattern


- Client doesn’t feel difficulty in falling asleep Sleeps at around 6-7 pm. They rest comfortably during
their shift and are light sleepers at times. Wakes up between 5 and 8 am and cannot fall back asleep once
awakened.

6. Cognitive – Perceptual Pattern


- She has no hearing loss and is not using any hearing aid.. 20/20 vision without the use of glasses.
Observed good short-term and long-term memory.

7. Self-Perception Pattern
- She described herself as someone who’ve full filled her role and duty, a hard worker who eventually
got exhausted. Although she has her condition, she’s trying to do her best to recuperate and make time for
herself.
8. Role – Relationship Pattern
- She was the third child - she could recall clearly, Currently the only one living out of her siblings, And
had a strained relationship with her husband.

9. Sexuality – Reproductive Pattern


- The patient is a woman Not currently engaging in sexual activity. Had a previous operation to remove
the the removal of left ovary and undergo a a raspa procedure.

10. Coping and Management Pattern


- Mostly prays and listens to conversations or converses with the staff and students. Finds herself
occupied with the routine making herself look presentable and just fullfilling physiological needs.

11. Value – Belief System


- Client is Roman Catholic prays, and attends mass depending on her free time and attends with her
being or attends alone.
