Informed Consent Form for Research Interview

Thank you for considering participating in this research interview. This document explains the
purpose of the study, what participation will involve and allows you to give your informed
consent.

The aim of this study is to know your perception towards guidance and counseling . Your
participation would help us better understand the students percetion towards guidance and
counseling

If you agree to participate in this research interview, we will conduct a one-on-one conversation
that should not take more than an hour. During this conversation, we will ask questions about
your basic information, Do you have any experience in guidance and counseling?, How do you
view the role of guidance and counseling as a student?, What are the factors that hinders you
from approaching school counselor in terms of your internal and external problems?, In what
situation you find guidance and counseling a necessity on your life as a student?, and As a
student, what are the things you can suggest to improve the effectiveness of utilizing guidance
and counseling in schools?

There are minimal risks associated with participating in this research interview; however,
confidentiality may be compromised if personally sensitive information is discussed during our
session.

You won't directly benefit from taking part in the study but future generations could benefit from
uncovering specific insights about guidance and counseling.

Participation in any aspect of this project is voluntary and participants have complete freedom
to withdraw at anytime without consequences.

All details disclosed during our conversational sessions will remain private and confidential as
only essential personnel involved with my academic supervision
