depose and state that:

1. We are living together as husband and wife since May 9, 2009 even
though we are not married;

2. Since we started living together, we never got separated. Even though we

were not married, we managed to preserve our relationship because of our love for
each other;

3. We executed this affidavit to attest to the veracity of the foregoing

statements of fact and for whatever purpose this affidavit may duly serve.
