- he denies any history of previous hospitalization for respiratory illness is no COPD asthma a TB

RT LUNG NODULE UNDER EVALUATION

ADVICE
- I have suggested them to cancel Pulmonologist at Rabindranath Tagore Institute in Kolkata as
he might need biopsy of right upper lung nodule or other optionis to vist NH Bangalore for
further management. It might take 1-2 weeks to establish the diagnosis

- I have also informed them to to get the CT CD chest review

- to consult Internal medicine/Endocrine physician for faitiguability with poor control of DM

- to inform Shilpa 95138 40601
