WAIVER OF RIGHTS AND ASSUMPTION

I, Lucia Omallao, of legal age, married, Filipino and with residence address at
Lanao, Kidapawan City, under oath, hereby state:

1. That I, LUCIA OMALLAO, I’m the registered owner of Electric Meter No.
1973689, under the account number 1010441150, for my residential building
situated at Purok Pomelo, Barangay Amas, Kidapawan City.

2. That by these presents, LUCIA OMALLAO, hereby, WAIVE, CONVEY and

TRANSFER all my rights, interest and participation over the said Electric Meter
bearing Meter No. 1973689, under account number 1010441150 in favor of
GLORIA B. PEJANER.

3. That I, GLORIA B. PEJANER, assumes responsibility of punctual payment of

my monthly Electric Bill for the aforementioned meter.

4. That we are executing this affidavit to attest the veracity of our foregoing
statements and to support the petition of GLORIA B. PEJANER with the
Electric Meter to transfer to her name bearing Meter No 1973689, under account
number 1010441150 and to become the absolute owner thereof.

5. That I am executing this Affidavit to attest to the truth of the foregoing statement
of facts.

bearing Meter No. 003984-18, under account number 03032070-0303 in favor of
GLORIA B. PEJANER.

3. That I, GLORIA B. PEJANER, assumes responsibility of punctual payment of

my monthly Water Bill for the aforementioned meter.

4. That we are executing this affidavit to attest the veracity of our foregoing
statements and to support the petition of GLORIA B. PEJANER with the
Water Meter to transfer to her name bearing Meter No. 003984-18, under
account number 03032070-0303 and to become the absolute owner thereof.
