Cebu has requested permission to hold a “Thanksgiving Party” with the following details :

Date : November 9, 2020 Time : 5:00 P.M – 9:00 P.M

Place/Venue : Residence in Sto. Rosario, Suba, Bantayan, Cebu

No. of Guest : 20 persons (see attached list)

After checking the venue and certifying that it can accommodate the guests with adequate physical
distancing (MR./MRS./MS.) MRS. LIA MONETTE T. ANIMAS is granted permission to hold this
event as he/she/they ) as promised to abide by the health and safety protocols set by the IATF and the
Municipality of Bantayan’s EO # 39 series of 2020, regarding the holding of social gatherings during
the quarantine period.

Attached herewith is his/her promissory letter stating the aforementioned person as paid the Mayor’s
Permit fee at the Municipal Treasurer’s Office, and has secured a Mayor’s Permit allowing this event.
