SWORN STATEMENT

I, Rex I. Igoy, of legal age, Filipino citizen, and presently residing at Romulo Boulevard, San Vicente,
Tarlac City, after having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am the President of Judyrex Igoy Corp., a corporation duly organized under the laws of the
Philippines with principal office at #28 Romulo Boulevard, San Vicente, Tarlac City.

2. That said corporation was incorporated on July 17,2020 with SEC Registration
Number (2020070000912-05).
3. That actual amount of collections (as contained in the lease contract) was not received from
January 1,2023 to December 31, 2023;

4. That the following are the monthly collections for 2023 from Seven Eleven of Judy-Rex Igoy Corp.:
