AFFIDAVIT OF LOSS TITLE
I, Kimberly T. Miraveles, 23 of legal age and residing/with address at Purok Manga, Andrada Village,
Del Carmen, Iligan City 9200 Lanao Del Norte, under oath, state:

THAT on March 20 at Iligan Capitol College while attending classes for my last subject at AVR third floor
around 7:30 pm, I suddenly felt anxious because the Criminal Law book 2 or Revised Penal Code that I
borrowed same date at the Iligan Capitol College library is currently losing, the first thing that came up to
B my mind is the book cause a price aside from that the original copy of this book. Last thing that I
O remembered, is some of my classmates are inspecting the content of the book due to the upcoming midterm
D exam.

STATEMENT OF FACTS
THAT Revised Penal Code Book 2, has a color yellow cover text with maroon background, and black in
overall cover, approximately 355 pages, with a width and length of 6x9.

THAT I exerted diligent efforts to locate the Revised Penal Code book 2 but to no avail and I therefore
considered the same the same as lost and can no longer be found;

THAT I am executing this Affidavit to attest to the truth of thr foregoing facts and to use the same for
whatever legal purpose it may serve.
